Addendum: the Capcom example is a mod, but at least still has an attractive face to begin with. The Sony example, no one would even want to touch with a thirty nine and a half foot pole.

Capcom is dialing back the sexy. Sure, Street Fighter 6 has some sexiness but not as much as previous titles. Resident Evil after 6 dropped all of the sexy costumes (and a great deal of the humor too). That said, the Capcom women still look like women.

Capcom got rid of the "overtime" line, and instead had Ashley thirsting over Leon the whole game.
What was the "overtime" line? I've never played Resident Evil.
At the end of the game Ashley, the President’s daughter, asks Gov. Agent Leon if he wants to do some “overtime” i.e. have sex with her, and Leon immediately friendzones her.

You don't need to cover up the face, because that could easily belong to a man too, or at least a teenage boy. Even the sex scene does little to depict any femininity in Abby, since the abomination looks like it's taking it up the ass.

The faces on Capcom characters are more appealing than Western characters, but they still look like a downgrade compared to older Capcom games.

I'm not a huge fan of scanned characters, but I'd argue it's more the insistence on making escapism "realistic" that needs to fuck right off.
The games industry is not exactly a meritocracy at this point, but I'd be willing to wager that they know exactly what they're doing. Ugly assets happen all the time due to production problems and budget restrictions. Shouldn't happen with a primary character though. If a major character in a AAA game turns out ugly, odds are it's because someone wanted an ugly character.
Motion actors need not look like their characters, and in practice, seldom do. All face scans are post processed, most turn out just fine (when that is the intent). Structural alteration is doable, though skill dependent with the risk of a character looking uncanny - not immediately ugly (when that is the intent). Poor performance capture or processing can result in weird results - but that probably wouldn't explain why the rest pose looks like a mong, would it?
