As one commenter put it, the worm only ate part of RFK's brain. With Biden, it ate the entire thing.

He's not that big of a deal in all reality. He has no chance in the election and would just become a martyr anyway.
Also tapeworm in the brain is just an extremely unlikely and as far as I can imagine, uncontrollable, outcome of eating undercooked meat. Assassin's generally use tools that have better than 1/10000 chance of working.
Plus, it's South Asia. While they're apparently making laudable progress on the outdoor shitting problem, India is still horrifically unhygienic by Western standards.

The worm died in like 2011, it's old news. But the headline "JFK Jr has brain worm" is a metaphorical brain worm to make fewer people vote for him since he's pulling more from Biden.
That picture of RFK fits perfectly as well. They really want you to see him as confused and tired from poor health.
He's also one of the better Democrats.

The article literally tells you why. He was being deposed during his divorce proceedings in 2012. He was claiming that the injury left him with less earning potential, presumably in an attempt reduce how badly he was being divorce raped.
