The difference is what happens after they get that bill. Whites will suffer, try to pay it, get sent to collections or end up bankrupt. Because that's the correct way to deal with these ridiculous abusive medical bills. The hospital will get some portion of its money back regardless, because most whites aren't gonna just declare bankruptcy immediately. And because they've jacked up the prices so high to play their little game with insurance companies, getting some back is usually most of the "real" cost of it.
Blacks and illegals will do none of the above. They have zero to garbage credit scores, on paper own little to nothing that isn't government funded to begin with, and, in the case of illegals, will just disappear if you try to push them on it. The hospital will get nothing back and will eat the cost of Manuela and Shaniqua with their 9 malnourished kids constantly in and out. A cost that gets passed on to the generalized public to offset, because the doctors and corporate owners demand line never stop going up.
I haven't disagreed that this shit is overly expensive and all sorts of fucked up. But if we removed these particular parasites from the equation, it would be less so against us.
One of the non-maliciously done causes behind the prohibitive cost of medical coverages is the obscenely common practice of just not paying at all. Ambulances, ER, doctor time, just used and then the bill ignored entirely. Not partially until bankrupt, but not even a dime spent towards it. From people who've never held a job good or long enough to have any form of insurance to begin with.
As such, the medical industry raised prices to account for those losses and make themselves not have to lose at all. The same way grocery stores raised prices to offset increased shoplifting and all insurances raised to cover the uninsured motorist increase and other similar epidemics. They weren't going to eat it themselves, they passed it on to us.
These aren't problems whites aren't guilty of, but relatively its a drop in the bucket compared to blacks and both are a fraction compared to illegals in this regard. Without them alone, we'd probably see a substantially less egregious pricing problem.
Obviously this is one aspect of the entire pie, but it is part of that issue.

Many of the ails in our medical system comes from the retarded, the addicted, and the "lol I ain't paying that" making everything need to protect itself from getting screwed. So prices are raised because all the niggers and illegals will never pay and they need to compensate, pain killers require you to be dying to get one because addicts are constantly scamming, and specialists need a filter to keep out the hypochondriac/Munchausen/idiot.
Government involvement is the problem across both systems.
The benefit to ours still is that I have that option to go find someone still under the banner of capitalism to fulfill my needs and keeping their prices affordable, instead of having zero options beyond "ultra premium private in another country" like the socialized do.
Problem is that "specific" doctors often operate on referral only, and you aren't going to see him unless that guessing doctor agrees you need to.
Obviously big ones like dentist or eye are easy enough, but things like throat or surgery are going to tell you to come back with said referral before they talk to you.
The only doctors I've had good experience with are small building docs instead of hospital ones. The kind who offer things like in house "pay us 100$ a month for your family to have unlimited visits" insurance. A business whose entire operation can go bankrupt if they get low trust, so they actually operate as humans trying to make this work instead of invincible egoists whose paycheck is guaranteed.
Heck, my wife had been having a severe hip problem and after years of "real doctors" basically throwing their hands up, our guy just decided enough was enough and started spamming surgeons until we found one who'd get it done and then wrote something that classified it into something insurance would cover.
Like most things in America, the turning of a profession from a literal thing someone did into a giant corporation and detaching it from being a guy you know into a faceless entity has destroy basically any value it once had. Civ is one of those games where the beginning is always the funnest part, and by about half way most people lose the steam to finish. Something it shares with a lot of CRPGs and SPRGs, plus a few others.
You start at equal footing with the CPU and have an abundance of choices in front of you, with a lot of opportunity for organic and unique gameplay to emerge. Even if they are retarded or cheating, lots can happen. By the time everything stabilizes, all the open land is settled it just becomes a slog of either you've already won but need to wait 100 turns to achieve it, or the opposite.
You aren't really ever going to fix that. Even if the CPU get much better, then they will just roll you because they are AI making perfect decisions. Focusing on doing so is a losing fight, as any change to do so (as you described) ruins the game for most people who are playing it just for the "promise" of challenge that isn't fun.
The only solution would be to make multiplayer less cumbersome, so that people are actually willing to sit and play it to the end. But I can't think of a way to do that, even my own family never plays more than 2 sessions on any new map.
They aren't animals. They have brains and choices. They choose to come here, work these jobs, and often refuse to assimilate while doing so. Being a pawn doesn't absolve them of anything as all those are things they directly made the decision to do, they weren't kidnapped off the Savannah and brought here in chains.
And if you think they are mindless animals, then we should be skipping all this theater and just putting them down like we do with overpopulated animals.
People focus a lot on the DEI leader choices instead of the biggest thing that bombed the game.
Which was the "you change civs with every 'era'" mechanic that they ripped from Humankind (a failed civ clone no one liked). Something that forced them to decouple your leader from your Civ on every level, and made it impossible for even the leaders people liked to feel right.
Even if every other element of the game was up to snuff, that alone would have tanked this game. Because it goes against one of the very things people play these games for in the first place.
I think one of the differences here is that they didn't really become part of the industry to begin with. One of the reasons this game took so long is because they just kept working on their own at their own pace at it, instead of immediately spending the first game's money to lighten their workload and become a "real dev team."
