That would be a swing towards sanity. These procedures are problematic because a doctor is supposed to have a system of ethics that would prevent them from harming their patient to satisfy a madman's fantasies. It's up there with abortion, trans surgeries and non-reconstructive plastic surgery as things that should be impossible simply because no doctor worthy of the title is willing to be complicit in the act.

iirc people with BIID have something gone wrong with their peripheral nervous system and it's not sending all the correct signals it should, which results in the brain thinking "hey this body part shouldn't be here" driving the person mad. So far there isn't a medication to alleviate it.
I really don't agree with doctors agreeing to surgically amputate on people like this, especially when most cases result in the person becoming legally disabled (not this case, which is why the doctor agreed to do it) but often someone with BIID ends up doing some DIY amputation or mutilation. Maybe therapy can help a person cope with the sensations and urges, but it doesn't sound pleasant either way.

In that case, a girl who had a (fairly minor) face deformity got it corrected with free plastic surgery... That was in 2012.
There's other similar examples, but searching for them by keywords mostly brings up cases of people who've shot, burned, or had their face bitten off, and that's... Too much internet for my morning, tbh. Once you see those photos, they never fully leave your mind. I do not recommend.

You have to have a way to quickly arrest and convict everybody involved in the chain that lets somebody have a cosmetic amputation.
