It's a good thing George doesn't live in the UK, otherwise he'd have gotten a visit from the police for, and I quote, "being untoward against paedophiles".
It reminds me of reality. That's why it's so unsettling.
I don't think he just means the subject matter. I think it's like a semi-over the top intentionally grotesquely drawn art - it's supposed to make you uncomfortable.
It's really well done.
He used to make it a point to have at least one nightmare face panel in every comic. He seems to have moved away from that a while back, but his old faces are still unsettling.

The first panel illustrates an old custom that I remember from childhood: When shaking hands, you tickle the other's palm with your finger indicating a desire to have sex. It has a name but I forget it.

They tried using this comic in place of the ink blot on his followup comic on r/therightcantmeme. It made them look like they were even crazier as the character is saying not every gay person is a groomer. This comic was literally in response to kids in DC getting monkey pox.
