I never understood why you need 'gender confirmation surgery' if you become a woman if you say you are.
Note the change in terminology: sex change => sex reassignment => gender confirmation.
Leftists are happy to argue this one, actually. They consider sex and gender to be different, so a person's body can be male but their mind can be a woman's. Woman =/= female, man =/= male. Woman is gender, female is sex. Changing the sex to conform with the gender is not a contradiction for leftists, it harmonizes gender and sex.
If sex and gender are different and they were "real women" from the start, how would sexual surgeries "confirm" the gender they wish?
I think all the changes are supposed to make them more passing, so a greater number of outsiders will assume the claimed sex/gender instead of the original, and that's supposed to be better for peace of mind or something (that doesn't end up being true).
I don't think many leftists argue that gender and sex aren't "supposed" to be aligned, after all, they're the only ones promoting surgeries, hormone treatments, etc. I think they generally agree you can be a woman trapped in a man's body, and that treatment can "free" them so they feel more at home in their bodies. That alignment is the "confirmation."
And I agree, it doesn't make sense.

Gender was the polite term for sex until some pedophile in the 70s muddied it up.

That’s not entirely inaccurate. Humans have trouble thinking about a concept for which there isn’t a well-defined term. Leftists steal words in order to control how we think.
If there’s no word for the concept: adult human female (because they stole the word “woman” and applied it to a different concept) then you can’t have policies or spaces for adult human females; you can’t distinguish adult human females from adolescent human females for purposes such as consent - it’s a really powerful hex they’ve placed on us.
They do this with a lot of things.
If gender is a social construct, and nothing is inherently male or female, then how does putting on a dress or liking sports make you male or female? I can be a man and wear pigtails. I can be a woman and chew tobacco. Since none of it is determined by sex or gender, how could doing them change your sex or gender?

the answer to that is simple: their definition of man and woman is self-referential. "a woman is anyone who identifies as a woman" etc.
of course, another way to describe a self-referential word is "meaningless".

That is true, but isn't their entire point about "women with penises" that biological sex doesn't matter?
Also, the 'gender' ideas they have are also pretty contradictory. It's a social construct and it's what's in your head.

Also it used to be diversity => diversity and inclusion -> diversity inclusion and equality -> OH WAIT THAT SPELLS DIE CHANGE IT TO DIVERSITY EQUALITY AND INCLUSION -> well, we don't really want equal opportunity but we want equal outcomes even though per capita we are already overly represented but it's not like we haven't been lying anyway this entire time and it was never about equality so chance it to diversity equity and inclusion

Speaking of which, did you see Madonna at the WC? She's had SO MANY surgeries on that face it's unimaginable. She passed Michael Jackson ages ago. She's 'The Argo' now: every part of that boat has been replaced, is she really herself or not? 🤔
