I think all the changes are supposed to make them more passing, so a greater number of outsiders will assume the claimed sex/gender instead of the original, and that's supposed to be better for peace of mind or something (that doesn't end up being true).
I don't think many leftists argue that gender and sex aren't "supposed" to be aligned, after all, they're the only ones promoting surgeries, hormone treatments, etc. I think they generally agree you can be a woman trapped in a man's body, and that treatment can "free" them so they feel more at home in their bodies. That alignment is the "confirmation."
If sex and gender are different and they were "real women" from the start, how would sexual surgeries "confirm" the gender they wish?
