Really depends on the context and intonation. Sure, if you point at someone and are like あの女超美人! while pronouncing it as ona, then naturally just from context clues they are gonna get it instantly. But if you are mispronouncing something that easy that terribly then you are probably fucking up everything else too so the second context makes it even a little less blatantly obvious, people are going to be tripped up by it even if they understand after thinking for a second or two.
All the strawmanning and false equivalencies in the world don't make this not the case.

We're talking a consonant sound that is either 1/8 of a second, or 1/4 of a second. With a difference like that, context is everything. It'd be like hearing someone from India pronounce the first "r" in "February" or the second "c" in "Connecticut." You might incline your head slightly, but probably wouldn't even be sure you heard what you thought you heard.
Speaking of strawmanning, your entire position here is one, in that you're pretending this is some earth-shattering difference, when in fact it's barely discernable out loud. Sure, I agree with that and even gave a concrete example in my previous post.
This is where you go off the rails. It isnt in any way the same. In English, there aren't even any other words that sound like "ConneCticut" or "FebRuary." Japanese has way less sounds and homophones are much more common as a result. If you fuck up pronunciation, intonation, and the context also doesn't make it obvious, people would absolutely be stumped for a second or two at least.

Even in a general sense, jokes like that can land alright if the language doesn't have the exact sound in question, and the one you use in the joke is the closest thing. But in this case they do have the sound used in the joke and it isn't the same. I'm reminded of a comedy bit where the stereotypical dumb gaijin dude makes a joke by switching かわいい and こわい and all the Japanese people at the table look at him like he's a retard. Obviously nowhere near as severe in this case, but similar energy.

Well, it's really just a different purpose. MtF is useful for traumatizing girls, and therefore making them easier to radicalize. FtM is the end in itself of sterilizing the people by removing viable mothers.
I think it more comes down to women are already - on average - in line with The Narrative. Destroying a small portion of women is simply collateral damage. Men are - slightly, on average - not in line with The Narrative, and must be destroyed and rebuilt. Destroying masculinity is a huge win for the powers that be.
