I think that it's more like: "We know that at least 30% of the population is transgender. If you don't let these kids become transgender, that is a genocide of the transgender adults that are currently transgender kids because you will have geocided them into being cis."
There's also the possibility that he means it literally. Vaush has been claiming that the Republicans are going to pass legislation that will actually create roaving death squads executing transgenders.
It's the difference between a rhetorical trope and literal-minded insanity. Vaush is taking a metaphor literally, a big sign of either stupidity or illiteracy.
I'd bet that many "advocates" for trannies believe it literally.

Notice how easily a tranny or tranny sympathizer could take this comment as a literal call for murder, when it's convenient to do so.

To be fair, the US government did screw up a lot with regards to HIV. It's a good thing that measures were taken to ensure the guy in charge of that disaster was removed from all forms of power. It would have been terrible if he was left in the same governmental position for decades and (over the last few years) used that position to destroy the economy and begin a forced injection campaign across the entire country.
