Adults aren't normally afraid of being seen as childish because they don't do childish things and in any case aren't overly concerned with how others perceive them. But if an adult plays with a rattle and wears diapers then that is still to their shame and they ought to be ashamed - not because of how others will perceive them, but because they are being immature and foolish. Same thing if you waste all your free time getting worthless video game points instead of improving yourself or contributing to society.
