That's how Italy was as I experienced it (20ish years ago but still) especially Roma. Garbage piles, flies, and the stench of rotten fish and open drains.
New Orleans is the only city that smells worse.

The garbage problem in various Italian cities was actually the result of mafia extortion rackets and other scams, which included burying toxic waste on arable land.
That's why you should avoid buying produce from Italy. It might be laced with carcinogens.
You've been Paris Syndrome'd. I assume it wears off and you just accept it at some point. But like someone else said, just move to a rural area.

With Paris Syndrome, the Asians were said to have had mistaken expectations of certain cultural experiences when visiting famous foreign cities. But what if those cities actually have turned to shit over time? What if Paris and London are now majority non-white, filthy and violent, and no longer culturally French or British? What if San Francisco is now littered with homeless drug addicts, human feces, and used needless? When does Paris Syndrome become “the Asian tourists were right”?
