The peer-reviewed articles don’t state anywhere the cases described are fictional.
I was going to speculate that these fraudulent "Case Reports" may not have been peer-reviewed at all (Case Reports in medical journals are simply interesting anecdotes of an IRL case that a clinician writes up & publishes about a rare presentation or a disastrous result. They are very different than primary literature about actual observational or RCT studies).
But I stand corrected in the very 1st paragraph of reading the link, as you are correct that the fake submissions passed peer review.

Pretty wild that the editors now claim decades later that they purposely knew the hundreds of fraudulent case reports were published without a disclaimer for "teaching purposes".
Some of the commenters on the article note that this is likely facesaving lies, as TWBM archives show that the actual "fictional case" disclaimer was only added after Sept 2025.

Wallis eventually returned, but was a changed man. He was weepy & weak. He openly called out the haters & wrongthinkers speculating at the cause of his wife's tragic death. He kept sporadically posting half-hearted vids for years, but never to my knowledge ever addressed the cause. He eventually started dating a woman/fan from the US last I checked.
His community on YT & Reddit were ferocious in sweeping up any questions or speculation related to her death. They mass reported any other small YTubers who made vids about the topic. Just look at Firaxis & Civ7's release last year after almost a decade since their last IP.
They tried to change up the formula from their classic Sid Meier franchise dating back to the early 90s (decoupling historic leaders from their ethnic civs, forcing civ switching at the end of eras stealing ideas from the failed competitor Humankind, dumbing down gameplay to make it easier for casuals - getting rid of classic gameplay mechanics like workers, assigning citizens to work city tiles, etc).
While I think their experimental changes were crass and shortsighted, it's also a good example why most AAA devs and studios are cowardly & lazy.
The response to the expensive & undercooked release last Feb may have destroyed one of the most successful remaining PC IPs dating back generations.
