If they refuse, they're thrown in jail like someone refusing the draft. No free ride for women or, in this case, mental cases. If you're sane enough to vote, you're sane enough to work in a factory or go mop floors or go weave "war baskets" or something for the government to sell to finance the war.
In the US, the 26th amendment lowered voting age for men to 18 largely on the argument that it's unfair for a man to be old enough to be forced to fight and possibly die but not old enough to vote, establishing the link between voting and the draft. Except women also got their voting age lowered at the same time, because the world is full of simps.
