If you have the magic or the tech to transform your body at will, why the fuck would you be trans?! You want to be a girl, wave a wand, drink a potion or step into a medical station and boom, you are now a fully FUNCTIONAL woman. Same thing if you want to be a guy, be a cat girl, cyborg or even another race.
If you put trans in a fantasy setting, and I'm not just talking about 1 weird guy that loves to cross dress as that's always existed but an entire category, you are just an uncreative shallow writer.

The truly delusional ones need mental help. If a schitzo thinks that his toaster is telling him to kill people we don't validate those delusions, we make sure that he isn't a danger to society and ideally treat him so the delusions go away. Troonery should be treated no differently. Of course, those numbers will go way down if we ever get serious about preventing that sick cult from grooming people into thinking that they're the opposite sex to begin with.
Very true, it's just a shame the very institution of psychology was co-opted too long ago by malicious parties that it's barely scratched the surface still to help these mentally ill people. We are talking about an institution that considered lobotomy and electro-shock therapy the future of mentally health.
If technology was ever developed that could absolutely turn people into the opposite sex perfectly, and give them all the right looks, organs, biological processes, and DNA, the trannies will be the most outspoken against it.
It could make for a good story, in the "what were they smoking?" school of anime style stories. Some guy has bad autogynephelia, and so clones himself as a woman, with his memories intact, but he creates the "perfect" body, and so its brain is better-formed, and isn't damaged and have such mental conditions, even if it has memory of them. And so the new body has no interest in the old one, nor in fulfilling any fantasies thereof, but has been afflicted with actual dysphoria thanks to the original's selfish fetish, that can't be fixed because tech is at "grow from scratch", not "alter existing" levels. From that base, you could go horror, romcom, slice of life, even action, based on how the two interact.
