I don't remember this character's context, but I do know that cross-dressing men are played for laughs in a lot of Japanese stuff (and hell, western stuff as well, because men dressing up like women is universally ridiculous).
Can't have humor or levity on this subject though; has to be about a troon pity party instead.
Amusingly, in the past I wouldn't have cared much about them. When I'd see one I'd just think, "well, that's an unfortunate dude." Now that we've been subjected to a decade of this look-at-me nonsense, I think, "stay the fuck away from me, asshole. I'm not using your goddamn pronouns."
I do find it difficult to tell with the Japanese at times but it's becoming clear some of the mentality is edging it's way into their society. Back in the day trap jokes were a thing themselves sure? I was watching Astro Note a bit because I thought it might be entertaining plus milf waifu but there was one episode where the boy wants to crossdress and next thing you know the woman is doing a full throated defence and the characters agree to let him crossdress at home in private.
Back in the day it was definitely a joke or not that serious, don't know about this game but if it was made recently I think it's fair to find any LGBT or tranny stuff instantly suspect. I also hate a lot how they constantly try to drag child characters into it because it's obvious what they're doing, brainwashing the easily manipulated normies into thinking it's a totally natural thing for a child nevermind a teenager to be doing all of that. No it can't be ideological brainwashing from leftist teachers and reddit.
In all instances, no one celebrated these freaks. It was a point of mockery and a source of entertainment.
Compare to today where people are pretending biology is altered based on what people wear, that freaks are entitled to extra rights for being freaks, and that anyone who laughs and mocks is a mini-Hitler that needs to be murdered.

There was also cross dressing in the west.
