I don't think he just means the subject matter. I think it's like a semi-over the top intentionally grotesquely drawn art - it's supposed to make you uncomfortable.
He used to make it a point to have at least one nightmare face panel in every comic. He seems to have moved away from that a while back, but his old faces are still unsettling.
