The medical charting system used (usually Epic) allows nurses and doctors to put in “presumed unvaccinated”

This is going to be the case for any medical system. There isn't one that can magically know the patient's status, or allows only the patient to input data. The vaccinated/unvaccinated numbers are just made up.

And even if they ARE proven vaccinated, if it's within the two week grace period it doesn't count.

Yes. Most of the people being hospitalized and counted as unvaksed are either

Admitted for other reasons (broken arm, etc)
Older patients who had a bad reaction to shot 1 and decided not to get shot 2

The dishonesty is shocking
