On July 1, 2015, 32-year-old Kathryn ("Kate") Steinle was shot and killed while walking with her father and a friend along Pier 14 in the Embarcadero district of San Francisco. She was hit in the back by a single bullet.
A single-action cowboy revolver sure as shit doesn't go off TWICE by accident. You have to manually pull back the hammer after each shot. Once could be an accident, maybe, if it was already sitting around with the hammer back, but twice? Nope. There's some fuckery going on.
Nope. Sure, it probably didn't help. But the problem was letting a dumbass who has repeatedly shown he has anger issues and thinks he is above the rules have a live weapon (even if it was only loaded with blanks). $100 says that ultimately this was Baldwin pointing it at people and pulling the trigger to feel all powerful only to have it actually do what a gun is supposed to do when you pull the trigger.

This is an earlier event that happened a couple days prior, but it does demonstrate a pattern of poor weapons handling on set.
The Pedia's Google blurb:
Sentient bullets did it again
Apparently people don't, in fact, kill people. It was indeed the guns acting independently all along.

If they werent handled and loaded properly... methinks you know little about the training that goes in to onset firearms.
Well there's your problem right there.
