Serious answer -- breastfeeding doesn't always just work. It's not always totally instinctual, there may not be enough flow etc.

Now, before the present day, women would learn these things from their mothers, family members, etc. There were nursemaids -- women whose jobs was literally nursing other people's babies -- etc. There were many solutions.

Today, like everything else, the solution is expensive masters degrees, trade associations, and paying the hospital / whatever organization extra money. It's kind of sad.
