Everything I've seen of Ciri after that one trailer she has looked relatively fine, meaning it might have been the rare case of actually just a bad time for a screenshot (as shown by people only showing the one single image as well).

CDPR has a lot of red flags regardless (like Ciri's new lore) so you still shouldn't trust them, but that one seemed like a misfire.
