The entire purpose of "insanity pleas" was acknowledging that a person was so far gone they should have been locked up to begin with, and society was the one at partial fault for not catching that sooner.

But the locking up is the most important part, almost no one who gets an insanity plea should ever be let out again. Even if the medication works, they cannot be trusted to take it on their own and them failing to do so clearly has extreme consequences.
They should be locked up in some kind of, idk, inhospitable jungle bordered on at least 3 sides by deep ocean. And maybe an impassible desert on the 4th.
