A friend of mine had a family member who had a stroke. Bill came back $450,000. They didn't pay anything because there was no reason to. Importantly: it would have been to dangerous to. If they had started paying bills before they filed for bankruptcy, that would have been used against them in bankruptcy court. So they didn't. Given their situation, no one should do anything different unless their in the upper-class income bracket.
Outside of that, no, you don't go to the hospital because it's too expensive. You go to an Urgent Care clinic, you get prescribed to many drugs you don't need and are addictive, and then you just muddle your way through life. This is what everybody does when they don't have insurance. If you go to the hospital and you need a surgery, or a multi-night stay.
