I had a PPO insurance policy that let me see specialists on my own so long as they were in-network. Granted, the policy was really expensive (I didn't know any better and got shuffled into a equivalent policy as my parents' when I turned 26 by their insurance agent), and I only used it to see an ENT for a chronic nosebleed.
Most people will be on an HMO policy, which does require you to see a PCP to filter out the people convinced they have turbo cancer for everything.
