A single shot nuclear weapon is purely defensive. Even then you can only use it against a nation that has very few nukes. Given their limited facilities and no bombers it would be local.

It would almost certainly be stolen before being used indiscriminately. You'd have to be ridiculously fanatical to not understand that order would never be fulfilled and whatever group of guys is currently in "possession" of it are about to disappear.
