All of the above. Iran and the US have explicitly shot down civilian airliners by accident. I can't remember if Israel has, but I wouldn't be surprised if that could be on the table given the amount of missiles flying.
Israel did, it wasn’t by accident though, they did it deliberately as a “terrorist” preventative because the flight went over the Sinai desert. This was back in like the 70s, I don’t think they’ve done so since.
I haven't heard of this incident, but I wouldn't be surprised. If it was during the Yom Kippur War, they very well may have shot down a civilian airliner out of concern for it being a disguised Egyptian flight.
