It can happen, but the specific circumstances here make it nearly (but not entirely) impossible.

Even my gun-savvy uncle recalls one scare he had. Unloading a shotgun, he gave it 2-3 extra pumps after the last shell came out. Then he though "I put in 12 didn't I? It seems like only 11 came out" and gave it another pump. Out popped the actual last shell! Stuff happens, eh?
