BPD is part of the human condition and exists everywhere. It might not manifest cutting specifically as often in other cultures, but it would manifest self-harm very often in some fashion.

The middle east is a black box in many ways. I wouldn't expect women with cutting scars to be visible in middle eastern cultures when even showing a woman's ankle is haram.

You'd have never "heard of this" a few decades ago in the US, either, because it wasn't talked about. Mental illness is disturbing and makes people uncomfortable, so the natural reaction is to cover it up and keep it hidden.
