Honestly, I expect something along the lines of ditching canon and make female Space Marines, everyone is now black except Titus, some girl bosses to emasculate Titus. I admit I'm not that familiar with the 40k lore but I assume he would take orders from Adepta Sororitas. So a girl boss to peg him is in the realm of possibilities.
At the very least add a Sister of Battle that wanted to be a Space Marine but the Patriarchy did not allow her to fulfil her dream, she will be bigger and stronger then Titus and defeat him in a non-interactive scene. something retarded like that.
GW doesn't allow their lore to pissed on. Whenever Black Library are accepting submissions, they give guidelines that make it pretty clear that you won't get in if you're trying to rewrite their universe.
There's a reason the stakes in 40k videogames are always relatively low -the fate of some random planet in some random corner of the galaxy, usually- despite the options the 40k universe allows. It's because GW literally won't approve anything bigger. You're never defeating a chaos god, you're never saving Terra, you're never opening the terminus decree, you're just NOT doing anything outside the scope of what the fluff lays out as typical affairs.
But lets suppose they did cave and allow what you just described. That gigachad Sister of Battle - probably ascended to the state of living saint... Is still a tool of the emperor.
