She can't have been a very good instructor.
Have you ever flown a Cessna? 100 feet is really not high up. It sounds like they'd practically literally just taken off, and were trying to gain altitude. It takes a while to get comfortable with takeoffs and landings, and I don's know how many lessons the student had taken before the crash. Accidents can happen easily, which is why pilots receive such specialized training before being granted certification.
100 feet is really not very high up. When the plane is banking upwards, you lose some of your visuals, so if you're not paying strict attention, 100 feet is really not much time at all for an instructor to realize that you're losing altitude, that the student isn't properly correcting course, that you need to step in and to then push the throttle all the way in and lower the tilt of the plane. Especially if the student had successfully taken off numerous times before, giving the instructor confidence that he could handle it, so she might have been distracted planning the exercises or the course.
