I mean, the Japanese have had a lot of pseudo LGBT people in their anime for decades now. Gurren Lagann is beloved and it has a character that outright says he's gender noncoforming. One Piece has characters like this too and its the biggest anime of all time over there.
Yeah, it's either implied through subtext (aka Cardcaptor Sakura and the main character's obviously lesbian but wont admit it friend) or it's so fucking in your face it's meant to be over the top because it's funny.
They don't fucking advertise about how lgbtbbq their anime are.
Benthram's still one of my favorite characters though, doesn't matter that he looks like a drag queen. An absolute chad of the chads who doesn't give a fuck, can be anyone with his powers and I don't know anyone who did not like him after Impel Down.
Idk, I know what you mean, but I feel like there's a big difference between when its every now and then for some sort of comic relief versus shoehorned in all the time and 100% serious not in any way meant to be comic relief.
the amount of isekai manga with some batshit insane overly muscly man with a perpetual five-o-clock shadow who "identifies as being female" (or similar) and acts "girly" is quite high

Different nations and cultures have considerable different notions of what crossdressers and wierd in between gender shit are. Comparing drag queens to crossplay traps in Japan, despite being arguably the same, is completely useless.
