You are partially correct. They're Long Range Ballistic Missiles. Actual ICBMs can hit any target on the planet since they get up into orbit. They could (in theory) circle the Earth a few times before re-entry.
MIRVs are nice, but just a fancy (cheap and efficient too) way to deliver several small bombs.
Any nuclear-armed nation is technically in the MAD club. In theory: if any nation uses a nuke on any other nation? The offending nation will be destroyed by every other nation, especially by the nuclear armed powers. Probably using nukes, but other ways are possible too.
