I suppose they could be suggesting that the bullet hit so hard that trachea shrapnel fragments were displaced upwards & outwards.
But it is still anatomically very improbable. The trachea is quite high in the neck and directly in the midline. Most initial videos suggested that Kirk was struck on one side of his neck and much lower, likely hitting his jugular that is lateral to the trachea. Some initial reports had the strike be lower in his chest much closer to the clavicle, a fair distance from the trachea itself.
Even if the trachea were struck, it would still be unusual for fragments of it to be ejected from his mouth, as they would have to clear the narrow larynx and go around the corner to clear the tongue and the dentition.
