I envision "hell" as a place far away from God (I'm a Deist, eh?)
These people have made their own hell & are happy to live there. Suffering makes them happy! As does hate.
As for the afterlife? I think there's a variety of places souls can go to, Heaven being the best! It isn't perfect (sorry to wipe out the favorite Atheist argument!) but it's way better than Earth.
As I interpret it, Hell is just in the center of the Earth. Somehow the gravity keeps everyone sent there, and it is pure pain for all the demons and people who are sent there. From what I understand, officially it's a lake of fire, that is separate from God.
I imagine Heaven is beyond comprehension, and takes up the entire universe around Earth. I'm not sure if other afterlifes would exist, but I wouldn't think they would.
I think other dimensions exist, and Heaven and Hell are in them. We're here in the 4th dimension, and there could be "side dimensions" here too. Like Oblivion (The Void) or indeed Purgatory (where souls go to await their final Judgement, once their deeds on Earth have finished affecting others or the balance tips so far that the outcome is obvious).
I'm thinking God exists in the 7th dimension? Which is why we aren't capable of fully understanding Him, any more than an ant can understand Shakespeare. :/
