I believe in universe its a graft, not a replacement. Plus in universe cyber psychosis doesn't affect everyone the same way. Some people are more resistant than others, and Adam Smasher seems to be basically immune. Although that's probably because he was always a psychopath.
Heh I had a table top where the party encountered Adam Smasher, we didn't beat him but we survived him. He rolled a double critical failure when dodging one of our attacks and snapped off one of his own legs when trying to maneuver. We immediately ran after that because we set a bomb that was about to blow.
