Yep, a few years ago, in Canada, the questions were all about

A) Traveling to certain countries in certain time frames B) Contracting Diseases C) Working with monkeys D) Trading sex for money E) Being a homosexual man or having sex with a homosexual man.

They axed the part about being a homosexual a few years back, and the donation centers positively ooze leftism, so I don't do it so often anymore.
