If you want to be technical the person who trooned out did. The death happens the minute they sell their soul to become a troon. After that point it's just a demon wearing a human suit.
So the inevitable suicide is just the body catching up?
If they go all the way through with the puberty blockers and dick chopping then they become genetic dead ends as well.
I have an acquaintance that went full troon a few weeks after having twins. Those were the second and third. The (2) bio moms are mental cases as well. Sad.
Also being dead to your family if you troon out. At least in a normal family that isn't into delusions.
