It really makes me sad that the denial of reality has become so strong
They don't really believe it. But they have to pretend to in order to continuously prove their loyalty to the revolution.
This guy's a troon, so he likely feels that either he pushes this ideology or he gets locked in a madhouse.
Could he just... not troon?

Troon propaganda is so strong these days that they are genuinely convincing tomboys and skinny nerds that their below average femininity/masculinity means they're trans, and that if they haven't realized it, they're 'eggs'. Eggs are indoctrinated into the cult with mass love-bombing until the sense of belonging motivates them to go through with the initiation ritual and troon out.
Once the ritual is complete, you're most likely in for life even if you regret it, because leaving (detransitioning) is painful, humiliating, ostracizing, and most importantly, the physical effects of the ritual are impossible to physically, completely reverse.
It functions like a religion from top to bottom.
That's unironically what I expect the future holds.
They will treat refusal to agree with John Money's concept of 'gender' as some kind of delusion of cognitive deficiency.
They'll show you a troon and if you say what you see they'll literally have psychiatrists interviewing you about your inability to perceive gender correctly. They will pump you full of drugs until you'll say yes to anything.
Might take 10 years, might take 50, but that's where it looks like we're going at the moment.
