Weird thoughts are part of the human condition. Repudiating the fucked up ones is societies job, and society is failing miserably right now.

Now let's get back to the original point, because I'm going to state something blatantly obvious.
Old people aren't attractive. Therefore nobody will sleep with their parents.
Siblings don't like each other. Ever. I don't know a single pair of siblings that don't hate each other.
What normalizing this idea would do, is create people who pretend to be related to their boyfriend or girlfriend, which is already a thing. It'd just be more common. Hell, I used to know one. Not going into more detail than that.

I'd say less perversion and more just...necessity? You'll have a bunch of families settle throughout these hills in the Ozarks or Appalachians and, well, there really isn't much new blood coming in, so over the decades these families just end up marrying into each other. Repeat for long enough and everyone's related to everyone else.
