Since I don't know anything beyond that one panel and the description I'd have to say there's nothing necessarily wrong with this. Joker self impregnating and giving birth to his own son (I wouldn't trust anyone ELSE with my baby; not even his mother!) to wreak havoc with is not entirely out of line for someone who has no limits. "So sorry for poisoning the reservoir. I'm murdering for two now, don't you know!" "With me around, you should be planning your funerals and not parenthood!"
But it is current year, so in all likelihood it's the delivery (heh) that's being botched.
It's the "B side" of the issue in question as the main story is Joker going around a hospital of cancer patients while being chased by an ineffective Red Hood/Jason Todd who really should have smoked Mister J within the same second of seeing him.
B side plot is Zatanna casts a spell on Joker by saying "No one else will ever have your baby", backwards obviously, and the result is Joker "gives birth" the next morning to a sentient pile of mud that eventually forms into a likeness of his own.
