I once worked at a start up where 100% employer paid health insurance was a thing. Pretty nice perk. But about 1/2 of the people who worked there were smokers, and about 1/3 of them were overweight with some overlap on the Venn diagram. It was crazy, and taught me a practical lesson in "if you don't have to pay for it".
