Surgery done through the belly button or a similar small entry point to reduce scarring. Often the preferred method for removing the appendix these days rather than leaving a scar that can be several inches long on some along the pelvic girdle.

Can they even do a mastectomy as a keyhole surgery now? I Would think that process hasn’t been refined enough so they still use a traditional surgery.

I doubt it given the dramatic range of tissue removal required for some. The appendix is at least entirely internal and not that large so removal doesn't mean much changes in the local area but external features are a completely different situation.

Have always wondered what happens in large scale organ removal surgery like hemispherectomies where half the brain gets taken out or lung transplants from living donors that give one up. That's a lot of space to fill and what it gets filled with fascinates me.

Plenty of scar tissue and fluid fill the spaces in between in those situations. Body tries to repair itself according to an instruction manual that no longer absolutely applies.
