Just the basic fact that there isn't any negative feedback in the system to encourage people to stop receiving or reduce the amount of medical care they're receiving is a huge problem that causes healthcare costs to spiral out of control. It's more in the system's interest that an 800 pound man stay 800 pounds and receive all the treatments necessary to keep him alive at that weight than to tell him to lose 650 pounds.

Or that they had to wait X months for some procedure while there's free clinics for illegal aliens that for some reason we aren't deporting.
