As for the killing of Jews, that started in November 1938 with Kristallnacht, but the regime attempted to make that look as a spontaneous uprising (last year was more of a Kristallyear, though somewhat less deadly). From mid-1941, Jews started being killed by machine gun fire, but Reichsfuhrer-SS Himmler thought that this was too inhumane... for the killers, so alternatives were sought and found. The decision was apparently taken in January 1942.
