There was no repeal and replace. There were no anti monopoly investigations, no improvements, no meaningful changes. After the Paul Ryan/John McCain fiascos Trump moved on to other interests and the whole healthcare mess was ignored.
Until it lit a fuse years later. And then his re-election campaign was completely undermined by a public health crisis which spread through every level of civil life and the economy and bureaucracy and led to last minute mail in voting changes.

Yep, and he had several right wingers who were representatives and Senators who were loyal MAGA types yelling in his ear to do something about the party of Davos and media. Trump focused on the wrong things. Some GOPers tried but all they did was yell at CEOs that don't give a fuck about that.
I suspect it's because he's too much of a boomer to understand how bad of an impact ALL of social media has made on the political conversation.
