I was going to say Elden Ring, but we mean Western games so nvm. Space Marine II would probably be what comes to mind.

I mean, one could argue the main antagonist was just a big trannie. Oh uh.. spoilers. I guess. Then the DLC got reeealy weird with it with their big bad. Like, really fucked up if you take more than 5 seconds to think about the implications. But it gets a pass, because Elden Ring is fucking amazing.
