I agree with you in principle, but culturally I’m pretty sure that the roles are a little different and so the distinction might be valuable. For instance, I think most Thais understand that “lady boys” are men with a fetish—there’s not an equivalent cultural lobby there pushing that “NO, THEY’RE REAL WOMEN!”
I hate to "akshully" but this shit is part of their culture, like it dates WAYYYY far back. Something about spirits and shit. There's no social movement pushing for their acceptance - they're just kinda there.
