Oh my goodness I do not remember it being this much of a chore to play. Crashing at various loading screens, half the time it doesn't save beforehand. The disk is clean, my console doesn't have any issues with any of the other games I have. Is this a specific problem to BO2, an age issue or did Xbox send out some update recently that breaks games? I don't have access to another disk to see if it's a hardware issue.

How did you store your disk? If you kept it in a hot attic, that could cause it to degrade.

The campaign or online? I only played the campaign very close to launch. I don't remember any issues, but it's been a long damn time. The multi-player, well I'm a bit ashamed to say I'm the max level and I never remember a problem crashing.
The campaign, never played much multiplayer. Not sure of the issue.
Normally I would, but I don't have a computer with the power to run 360 games since emulation for the console isn't as well developed as older models. Trust me, I've tried lol.
The ps3 emulator (RPCS3) can run smoothly on a cheap $250 minipc, all you need is a relatively good cpu which a lot of these things have

I don’t know how Xenia (x360 emu) is these days, but I was playing MGSV, Skate3, and CoD AW - with an Xbox controller and upscaled to atleast 1080p (from 720p native), all games I missed out on at the time too which was great, and they looked as good as anything I play on my PC these days. MGSV especially was impressive given they achieved it 3 generations ago

You could try installing it to the hard drive and see if that makes a difference.
I played the PS3 version years ago, and never ran into those issues.
BO2 was my second favorite cod after MW1. I don't remember it having any of these issues
