"After hitting head" suggests that the "man" fell down, "hitting [his] head." It appears to be passive voice because the headline does not name an actor, like "Jagoff leftist shoves old man, who hits head on pavement." Need to read the item to make sense of it.
I don't think the headline has passive voice. The tweet uses passive voice. I understand passive voice to simply be when the subject performing the verb is placed after the verb. If A hits B, passive voice would phrase it 'B was hit by A.'
Yes, passive voice is contextless grammatical structure. But writing "passively" is a value judgment on a piece of writing.
