A friend of mine who I have known for 20+ years decided recently he was a woman.
If he speaks to me about, say, cooking I will listen to him if what he says makes sense on its face.
If he speaks to me about what it means to be a man or woman, he has obviously ceded any ability to be taken seriously due to a serious (and unfortunate) malfunction in his person.
Even if he was telling me "yeah that guy's faking it", well I think you're faking it, so I'm not really sure what to do with that information other than applying it in equal measure to his situation.
I've only seen him once after he decided all this, so I'm not exactly sure what his expectations are. Though he changed his name and everyone else calls him "she", so I'm guessing there's at least a soft expectation.

Does he expect you to added him as a woman?
