A lot of it depends on manual reports, but I don't think that's true as of the last 6 months, since Reddit is using bot scanning text to automate a lot of bans now.
I've seen people saying tranny in definitely the last 6 months. They claim they are not getting banned when I mention that I have been. I haven't exactly followed up on each of them to make sure. A lot of people avoiding the word and saying transformer too. IDK
I think the admins mark certain subs and accounts for extra scrutiny.
The account I got banned for is pretty new, so I think what was happening is an admin took a sweep through a subreddit and did some banning but then they don't repeat that consistently. Thus other people were able to come into the same subreddit and do the same thing without a ban... yet. I will say that no matter what happens it takes them an internet FOREVER to do anything about it. At least a week.
Twitter is a little bit faster but kind of the same. Your Tweet will be allowed to hang out a long time before they get around to making you take it down.
I have been banned by an automated system a bunch of times this year, maybe a dozen or more. I got most of those reversed on appeal.
Manual bans never get reversed on appeal.

Using that site to launder any words I use that in any way relate to violence, mental illness, etc, has reduced my ban rate.
